Who's eligible?

The Diabetes Management Program by Teladoc Health is available to all FEP Blue Standard® and FEP Blue Basic® members who've had at least one medical or pharmacy claim in 2025 or 2026 with a diagnosis of type 1 or type 2 diabetes, or pre-diabetes.

Members living overseas must have a valid APO, DPO or FPO address to participate. If you’re overseas and have questions, you can call Teladoc Health support toll-free here.

To register a dependent under the age of 18, a parent or guardian must first create their own account and then add the dependent using the "Account" menu.
